This is only possible with cooperation of patients and implementation of medical recommendations. The training in heart disease cause in achieving empowerment and patient compliance with diet therapy is desirable. Objective: The present study aimed to determine the effect of cognitive-social based care program on adherence to treatment in patients undergoing heart valve surgery in military hospitals. Materials and Methods: It is a semi-experimental study done on control group in a military hospital in Tehran. In the present study, thirty patients referring to the military hospitals were selected by non-random sampling method and divided into intervention and control groups through pseudo-randomization method. Data were collected by valid tools for adherence to treatment in chronic disease. The control group received only the usual care and the intervention group received a cognitive-social care program. Immediately after the intervention and twenty days later, data were recollected. Descriptive and inferential statistics were used to analyze the data using SPSS 16. Results: In the present study, with regard to the adherence to treatment before the intervention in all areas of treatment, willingness to participate in treatment, compliance ability, combination of treatment with life, adherence to treatment, commitment to treatment and uncertainty about the treatment there was no significant difference between both groups based on independent t test(P>0/05). However, at the time of discharging and twenty days after that, differences in all areas were significant between the two groups (P<0/05). The results from the analysis of variance with repeated measures within the cases (F= (2.56) = 32.14 (P<0.001)) indicated that there was a significant difference in adherence to treatment with regard to time index at different stages. Analysis of variance with repeated measures between the cases with group index (F= (1.28) = 32.85 (P< 0.001)) showed that the effect of different times on treatment adherence in both groups was significantly different. Discussion and Conclusion: The results of this study showed that the implementation of training based on cognitive-social theory of Bandura strain improves the treatment compliance in patients undergoing heart valve operation.
