Effect of Cognitive-Social Based Care Program on Patient’s Compliance in Heart Valve Surgery in Selected Military Hospital

Introduction: Controlling chronic diseases, such as heart disease, depends on the extent of adherence to the treatment. This is only possible with cooperation of patients and implementation of medical recommendations. The training in heart disease cause in achieving empowerment and patient compliance with diet therapy is desirable.
Objective: The present study aimed to determine the effect of cognitive-social based care program on adherence to treatment in patients undergoing heart valve surgery in military hospitals.
Materials and Methods: It is a semi-experimental study done on control group in a military hospital in Tehran. In the present study, thirty patients referring to the military hospitals were selected by non-random sampling method and divided into intervention and control groups through pseudo-randomization method. Data were collected by valid tools for adherence to treatment in chronic disease. The control group received only the usual care and the intervention group received a cognitive-social care program. Immediately after the intervention and twenty days later, data were recollected. Descriptive and inferential statistics were used to analyze the data using SPSS 16.
Results: In the present study, with regard to the adherence to treatment before the intervention in all areas of treatment, willingness to participate in treatment, compliance ability, combination of treatment with life, adherence to treatment, commitment to treatment and uncertainty about the treatment there was no significant difference between both groups based on independent t test(P>0/05). However, at the time of discharging and twenty days after that, differences in all areas were significant between the two groups (P<0/05). The results from the analysis of variance with repeated measures within the cases (F= (2.56) = 32.14 (P<0.001)) indicated that there was a significant difference in adherence to treatment with regard to time index at different stages. Analysis of variance with repeated measures between the cases with group index (F= (1.28) = 32.85 (P< 0.001)) showed that the effect of different times on treatment adherence in both groups was significantly different.
Discussion and Conclusion: The results of this study showed that the implementation of training based on cognitive-social theory of Bandura strain improves the treatment compliance in patients undergoing heart valve operation.

Objective: The present study investigated the effect of interaction skill training and anger management on nurses’ exposure to aggressive behaviors of patients admitted in psychiatric wards of selected military hospitals.
Materials and Methods: This quasi-experimental study was a randomized clinical trial conducted in selected hospitals of Aja University of Medical Sciences during the years 2018-2019. Fifty-eight nurses were selected by census method and studied in control and test groups. The teaching materials included lecturing, using PowerPoint and Q&A in eight sessions. The effect of training anger control and interaction skill before and immediately after the intervention and two months after the intervention was assessed using a standard questionnaire. Data were analyzed by SPSS 20 using independent statistical t-test, Fisher’s exact test and Friedman test. Significance level was considered P<0.05.
Results: The result from Fisher exact test in both groups showed no significant difference in terms of reaction to violence before the intervention (P>0.05), immediately after the intervention (P>0.05) and eight weeks after the intervention (P> 0.05).
Discussion and and Conclusion: After the intervention, nurses showed a more favorable reaction to aggression than before the intervention, although these changes were not statistically significant. It seems that making a great change in this area requires more prolonged intervention. Thus, it is recommended that training interaction skills and anger controlling be included on the agenda of nursing managers with other training methods. Further studies with a higher sample size are recommended.

Introduction: The mental disorder in one member of a family cause a considerable tension in whole family. The mental burden imposed by caring mentally- ill patients can have a negative effect on the mental and physical status of care takers, thus cause a hidden illness and poor quality of life.
Objectives: The aim of this study was to investigate the effect of education the Basis of Roy Adaptation Pattern on quality of life of home caregivers of mentally ill patients.
Materials and Methods: This study was a randomized controlled clinical trial cperformed in 2018. The number of 50 home caregivers of psychiatric patients with a history of hospitalization in military hospitals in Bushehr was selected using convenience sampling method and then randomly divided into test and control groups. The home caregivers of the test group received a five -training -session of 90 minutes for adaptive behavior based on the adaptive pattern. Caring quality was assessed by Sf-36 quality of life questionnaire before and one week after intervention. Data were analyzed using Chi-Square, Fisher’s exact test, paired t-test and independent t-test in SPSS 22.
Results: Based on independent t-test, the mean of total quality of life before and after intervention was not significant between the two groups (P>0.05). In contrast, the results of paired t-test showed a significant difference in the test group before and after the intervention (P<0.05. In the control group, the results of paired t-test before and after the intervention were not significant (P>0.05).
Discussion and Conclusion: The results of the study showed that education based on Roy adaptation pattern affects the quality of life of home caregivers of mentally ill patients.

The Effectiveness of Social Competence Training based on Felner Model on Communication Skills and Social Adjustment of Nurses in the Psychiatric ward of Selected Hospitals of Aja

Introduction: Communication skills and social compatibility have long been recognized as an important part of medical and nursing care. Nursing, due to its feature, requires high social compatibility, specially nurses in psychiatric ward whose main responsibilities are to create a relationship based on trust and motivation in patients to participate in the treatment process. In the Fellner Social Competence Model, various components and dimensions of social adequacy are identified and, accordingly, the necessary training is provided for the development of social skills.
Objective: The present study aimed to investigate the effectiveness of Social Competence training based on Felner Model on Communication Skills and Social Adjustment of nurses in the psychiatric ward.
Materials and Methods: This study was a quasi-experimental study. Sixty selected nurses in the psychiatric ward of the AJA hospitals (in 2018) were included in the study and equally divided into the experimental and control hospitals. The experimental group was trained in a two-month 10 sessions, each 60 minutes based on Felner Social Competence Model and Barton Communication Skills questionnaire and Bell Social Adjustment questionnaire were used for data collection. Then, data were analyzed based on descriptive and inferential statistics using SPSS 25.
Results: The result of independent and dependent t tests showed that Social Competence training based on Felner Model significantly improved the mean scores of Communication Skills and Social Adjustment after the intervention (P<0.05). A precise examination of the sub-components of communication skills showed that in all of the sub-components, the scores increased after the intervention but this increase was only significant for the sub-component of listening skills (P=0.01).
Discussion and Conclusion: Felner Social Competence training is an effective way to improve the Communication Skills and the Social Adjustment of nurses in psychiatric ward. Thus, experts and health professionals can use Felner Social Competence training as an effective way to improve the Communication Skills and the Social Adjustment of their nurses in the psychiatric ward.

The Effect of Multimedia Training Program on Anxiety in Patients Admitted to Chemotherapy Departments

Introduction: Patients-with cancer in chemotherapy wards, face high degree of mental tensions such as anxiety in disease. These patients require effective training methods to manage anxiety. Multi-media education is one of the most useful and flexible educational strategies.
Objective: Thus, the present study aimed to investigate the impact of multimedia education program on the level of anxiety in patients admitted to chemotherapy wards.
Material and Methods: The present study is quasi-experimental and was performed on 30 patients with definite diagnosis of cancer referring to selected AJA hospitals in Tehran. Sampling was done randomly. The researcher gave educational CDs to the intervention group and no intervention was performed in the control group. The Spielberg’s Anxiety Inventory contained 20 multiple choice questions. Finally, the data were analyzed using SPSS software 16.
Results: The results showed that there was no significant difference between the mean anxiety scores before and after the test group (before: M= 43.96±6.05, after: M= 43.40±5.95) and the control group (before: M= 45.13±10.06, after: M= 47.53±10.32) (P> 0.05).
Discussion and Conclusion: The results of the present study showed no significant difference between the mean anxiety scores before and after the test group, but it is recommended to perform further comprehensive studies.

Introduction: Rationality and ethics are essential in the jobs which serve the people. The nursing with many moral aspects plays a key role in implementing a health care system.
Objectives: The aim of this study was to determine the effect of professional ethics program (in person and e-learning) on ethical performance of nurses in the emergency ward of armed forces -dependent hospitals..
Materials and Methods: The present study was a semi-experimental and pre-post-test study done on a control group in 2018 in three stages including before the intervention, after intervention and one month after intervention. Three groups (Control, face to face training and E-learning) were evaluated using the questionnaire of nurses’ ethical performance.A one-day workshop for in-person group and Shahab site e-learning for virtual education were arranged.. In the present study, sample population included nurses in the emergency ward of two selected armed forces hospitals in Tehran who are eligible and have inclusion criteria. After filling in the questionnaires, the data were analyzed in three stages using SPSS 25 (P<0.001).
Results: The results of the study showed that there was no significant difference in the ethical performance of nurses before the intervention in the pre-test between the three groups. While there was a statistically significant difference in the in-person training and virtual education groups in the first and second post-tests, but a higher statistically significant difference observed in the in-person training group.
Discussion and Conclusion: Considering the positive effects of professional ethics training to improve the ethical performance of emergency ward nurses, it is suggested that different training and planning methods be used to improve the status quo. However, based on the results of the study, in-person training is prioritized.

Intruduction: The thesis is one of the most important parts of the teaching of medical students and postgraduate student’s education. Thesis supervision is creating a learning agreement between students and supervisors and clarifying the roles of students and supervisors.
Objective: The success of students in acquiring research experience is one of the main goals of the thesis, and students’ satisfaction assessment is one of the most important challenges of that. So, this study was conducted to determine the satisfaction of postgraduate students and Residentss of Iran University of Medical Sciences from directing and supervising theses.
Materials and Methods: In this descriptive-cross sectional study, 250 Medical Residents and Postgraduates Students from different steps of thesis conducting were questioned. Data ghathering instrument was the PREQ standard questionaire that it’s validity and relability was confirmed. The data was analysed with use of SPSS21. The questionnaire examined six domains of the thesis. Kruskal-Wallis test for data analysis and Mann-Whitney test for comparing of means differences was used.
Results: In this study, 56.5% of postgraduates and 63.8% of medical residents expressed their satisfaction from the thesis supervision. There was no significant difference between the mean satisfaction scores in all domains in both groups. Supervisor professor, development of research skills, and thesis test earneed the most satisfaction score; and providing conditions and fields of thinking growth, infrastructure and facilities domains acquired the lowest satisfaction score.
Discussion and Conclusion: Findings showed the satisfaction level of both groups was relatively desirable. As regard that Significance difference was observed between proposal confirming, providing conditions and fields of thinking growth, development of research skills and clarifying of criteria, and goals with students’ satisfaction, attention is needed to increase the skills and enhance the ability of students to conduct independent research, in the early years of postgraduate education.

Nursing Bachelor’s Education Program in Iran and UCLA: A Comparative Study

Introduction: Considering the world’s most successful educational systems is an indication for the institutionalization of an effective system and comparing different educational systems will promote the content and quality of the curriculum.
Objective: The aim of this study was to conduct a comparative study of nursing Bachelor’s Education Program in Iran and University of California, Los Angeles (UCLA).
Material and methods: This descriptive-comparative study was performed in 2018, data were collected by googling the key words of nursing, nursing education, curriculum, California UCLA and Iran, and a review of the history and details of the nursing Bachelor’s education program in Iran and UCLA and analyzed using Bereday’s four-stage model including description, interpretation, juxtaposition and comparison.
Results: The goals, missions and structures of both programs are similar. Considering the characteristics and cultural diversity of patients, the patient-centered approach, caring for the high quality and safe care are the goals of the UCLA Nursing Faculty to the, which is less common in Iran. There are some differences in the pre-requisite courses.. In UCLA Nursing School, clinical education is done in the simulator lab equipped with the most updated educational technology, but in Iran, it is conducted in a clinical setting, using modern educational approaches is less common in Iran. Graduates at UCLA are not allowed to work until getting RN (Registered Nurse) license, but they start working in Iran after graduation.
Discussion and Conclusion: Considering that the program of nursing undergraduate studies in Iran was revised in 2014, performing descriptive studies on acquired competencies and program content can help to improve the program by problem solving.
