fa بررسی وضعیت اجرای دستورالعمل‌های مراقبت پرستاری ایمن از راه هوایی بیماران تحت تهویه مکانیکی در بیمارستان‌های نظامی Evaluation of Implementation of Airway Care Safety Guidelines for Patients under Mechanical Ventilation in Intensive Care Unit (ICU) by Nurses پرستاری داخلی - جراحی Internal-surgery Nursing پژوهشي Research <div style="text-align: justify;">مقدمه: مراقبت از بیماران تحت درمان با تهویه مکانیکی به خاطر عوارض ناشی از آن از اهمیت ویژه‌ای برخوردار است. هدف: مطالعه‌ی حاضر با هدف تعیین وضعیت اجرای دستورالعمل‌های ایمنی مراقبت از راه هوایی بیماران تحت تهویه مکانیکی توسط پرستاران انجام شده است. مواد و روش‌ها: این مطالعه توصیفی در بخش‌های مراقبت ویژه بیمارستان‌های منتخب نظامی شهر تهران در سال 1399 انجام شد. جامعه مورد مطالعه کلیه پرستاران شاغل در این بخش‌ها بود. بر این اساس عملکرد 90 پرستار چهار بار، مورد مشاهده قرار گرفت. در مجموع 270 مشاهده از عملکرد پرستاران در مراقبت از راه هوایی بیماران، در چک لیست مشاهده‌ای محقق ساخته، ثبت گردید. داده‌ها با استفاده از نرم‌افزار آماری SPSS نسخه‌ی 25 و با کمک آزمون‌های آماری توصیفی و استنباطی مانند آزمون تی مستقل، آزمون آنووا (ANOVA)، ضریب همبستگی پیرسون تجزیه و تحلیل شدند. سطح معنی‌داری کمتر از 0/05 در نظر گرفته شد. یافته‌ها: 1/61 درصد نمونه‌های مورد پژوهش مرد، 92/2 درصد با مدرک کارشناسی و میانگین سنی 8/44±33/36 سال بودند. میانگین و انحراف معیار مراقبت ایمن در بار اول مشاهده 7/59±35/08، در بار دوم 7/30±12/35 و در بار سوم 7/05±35/61 بود. کیفیت مراقبت در هر سه دوره زمانی در حد نامطلوب بود. انجام مراقبت ایمن از راه هوایی بیماران با متغیرهای جنس، تأهل، سابقه کار، نوع استخدام، شیفت و سابقه‌ی گذراندن کارگاه در مورد ایمنی بیمار، مراقبت‌های ویژه و تهویه مکانیکی ارتباط معنی‌داری داشت. بحث و نتیجه‌گیری: در این مطالعه مشخص شد وضعیت اجرای دستورالعمل‌های ایمنی مراقبت از راه هوایی بیماران تحت تهویه مکانیکی توسط پرستاران در سطح نامطلوبی قرار دارد. نظارت بیشتر بر عملکرد پرستاران، آموزش کاربردی و با کیفیت در مورد چگونگی مراقبت از راه هوایی بیماران و اجرای دقیق دستورالعمل‌های ایمنی، اکیداً توصیه می‌شود.</div> Introduction: Caring for patients treated with mechanical ventilation is of particular importance because of its complications. The occurrence of various diseases, including pneumonia and its resulting mortality, is affected by inadequate and improper implementation of principled and safe instructions by the care team, especially nurses. Objective: This study aimed to evaluate the implementation of airway care safety guidelines for patients under mechanical ventilation. Materials and Methods: This descriptive study was performed in 2020 in the intensive care units (ICUs) of selected Military hospitals in Tehran. The study population included all nurses working in these wards. The performance of 90 nurses working in these wards was observed four times. A total of 270 observations of nurses’ performance in patients’ airway care were recorded in a researcher-made observation checklist. Data were analyzed using SPSS software version 25 and descriptive and inferential statistical tests such as independent t-test, ANOVA, and Pearson correlation coefficient. The significance level was considered less than 0.05. Results: In total, 61.1% were male, 54.4% were married, 92.2% were bachelor, and 7.2% had a formal contract with a mean age of 33.36±8.44 years. The total amount of care in the first time was 35.08±7.59, in the second time was 35.12±7.30, and in the third time was 35.61±7.05. Safe airway care of patients had a significant relationship with the variables of gender, marital status, work experience, type of employment, shift and work history of patient safety, intensive care, and mechanical ventilation. Discussion and Conclusion: In this study, it was found that the status of implementation of airway safety instructions by nurses for patients under mechanical ventilation is at an unfavorable level.
