fa بررسی تأثیر مدل مراقبت پیگیر بر خودکارآمدی بیماران تحت همودیالیز در یک بیمارستان منتخب نظامی در تهران Investigating the Effect of Continuous Care Model on the Self-Efficacy of Hemodialysis Patients in a Selected Military Hospital in Tehran تخصصي Special پژوهشي Research <div style="text-align: justify;">مقدمه: همودیالیز و مشکلات ناشی از آن، تغییرات زیادی در زندگی این بیماران ایجاد می‌کند که نیازمند مشارکت خود بیمار در انجام مراقبت از خود در روند درمان می‌باشد. خودکارآمدی در بیماران تحت همودیالیز منجر به خودمدیریتی مناسب‌تر مشکلات می‌شود. هدف: این مطالعه با هدف تعیین تأثیر مدل مراقبت پیگیر بر خودکارآمدی بیماران تحت همودیالیز انجام شد. مواد و روش ها: پژوهش حاضر یک مطالعه کارآزمایی بالینی است که بر روی 74 نفر از بیماران تحت همودیالیز مراجعه کننده به بیمارستان شهید چمران شهر تهران در سال 1402 انجام شد. بیماران با روش نمونه‌گیری در دسترس انتخاب و به ‌طور تصادفی ساده با پرتاب سکه در دو گروه آزمون و کنترل قرار گرفتند. در گروه آزمون علاوه بر مراقبت‌های روتین، مدل مراقبت پیگیر اجرا شد. میزان خودکارآمدی بیماران قبل، بلافاصله و یک ماه بعد مداخله سنجیده شد. برای جمع‌آوری اطلاعات از پرسشنامه مشخصات فردی و پرسشنامه خودکارآمدی عمومی شرر استفاده شد. داده‌ها پس از جمع‌آوری در نرم‌افزار SPSS نســخه 23 وارد شــد و مورد تجزیــه و تحلیل قرار گرفــت. 0/05>P به عنوان ســطح معنی‌داری در نظر گرفته شــد. یافته ها: قبل از مداخله، دو گروه، از نظر مشخصات فردی و میانگین خودکارآمدی (آزمون: 4/49±36/67، کنترل: 5/44±34/62) اختلاف معناداری نداشتند (0/05<P). ولی از نظر میانگین خودکارآمدی بعد از مداخله، در گروه آزمون 8/37±62/35 و در گروه کنترل 2/18±34/13 بود که بین دو گروه تفاوت آماری معناداری مشاهده شد (0/001>P) و در گروه آزمون، خودکارآمدی ارتقاء پیدا کرده بود. در زمان پیگیری نیز، این تفاوت بین دو گروه معنی‌دار بود (0/001>P) و در گروه آزمون خودکارآمدی همچنان بالاتر بود. نتیجه‌گیری: با توجه به تأثیر مدل مراقبت پیگیر در افزایش خودکارآمدی بیماران تحت همودیالیز، استفاده از الگوی مراقبت پیگیر در بیماران تحت همودیالیز توصیه میشود.</div> <div style="text-align: justify;">Introduction: Hemodialysis its associated challenges significantly affect patients’ lives, requiring their active participation in self-care during treatment. Self-efficacy plays a critical role in empowering patients undergoing hemodialysis to better manage their condition Objective: This study was aimed to investigate the effect of continuous care model on the self-efficacy in patients undergoing hemodialysis Materials and Methods: This randomized clinical trial was conducted in 2023 on 74 hemodialysis patients at Shahid Chamran Hospital in Tehran, Iran. Participants were selected through convenience sampling and randomly assigned to intervention and control groups using a coin toss. In the intervention group, in addition to routine care, the continuous care model was implemented. Self-efficacy was measured before the intervention, immediately afterward, and one month later using a demographic questionnaire and the General Self-Efficacy Scale (GSES) developed by Sherer. Data were analyzed using SPSS version 23, with a significance level set at P< 0.05. Results: At baseline, there were no significant differences between the two groups in demographic characteristics or mean self-efficacy scores (intervention: 36.67±4.49; control: 34.62±5.44; P>0.05). After the intervention, the mean self-efficacy score significantly increased in the intervention group (62.35±8.37) compared to the control group (34.13±2.18; P<0.001). This improvement was sustained at follow-up; with the intervention group maintaining significantly higher scores (P<0.001). Conclusion: The continuous care model was effective in enhancing self-efficacy among patients undergoing hemodialysis.
