Introduction: Health literacy and self-care are significant factors influencing the quality of life of medical students. Objective: The present study aimed to determine the relationship between health literacy and quality of life mediated by self-care among students of Aja University of Medical Sciences. Materials and Methods: This study is a multivariate correlational research employing structural equation modeling (SEM). It was conducted cross-sectionally during the first half of 2024. The statistical population consisted of all students at Aja University of Medical Sciences. Based on rules of thumb, 297 individuals were selected via convenience sampling. Data collection tools included standard questionnaires on Adult Health Literacy, Quality of Life (SF-12), and Self-care. Data were analyzed using SPSS version 24 and Smart PLS version 3. Results: The mean score for health literacy was 77.28 ± 13.41, indicating a "sufficient" level. The mean self-care score was 34.26 ± 3.39, demonstrating a high level of self-care ability. The mean quality of life score was 53.38 ± 5.43, indicating a good status. Path analysis in the structural equation model revealed that both health literacy and self-care had a direct and significant effect on quality of life (P<0.001). Furthermore, the mediating role of self-care in the relationship between health literacy and quality of life was confirmed using the Sobel test (Z = 2.067, P<0.05). Conclusion: Health literacy and self-care not only directly affect quality of life, but self-care also mediates the relationship between health literacy and quality of life. These findings underscore the importance of health-centered education, the promotion of self-care skills, and attention to the psychosocial components affecting students' quality of life. Designing comprehensive interventions that simultaneously promote health literacy, improve individual skills, and reduce academic stress can play a significant role in enhancing the quality of life of medical students.
