Introduction: Military service can significantly affect individuals’ mental health and may increase soldiers’ vulnerability to self-injurious behaviors. Resilience training, as a strategy for promoting psychological well-being among military personnel, enhances adaptive coping skills and may play an important role in reducing such behaviors. Objective: This study aimed to determine the effect of resilience training on the prevalence of self-injurious behaviors among soldiers in a selected military garrison affiliated with the Islamic Republic of Iran Army (Aja) in Shiraz, Iran. Material and Methods: This randomized controlled trial was conducted in 2024 at a selected Aja military garrison. A total of 52 soldiers were recruited through convenience sampling and randomly allocated to either the intervention group (n=26) or the control group (n=26) using simple randomization. Participants in the intervention group attended eight resilience training sessions based on Henderson’s Resilience Model, delivered through lectures and group discussions. Data were collected using a demographic information form and the Klonsky Self-Injurious Behaviors Questionnaire at three time points: baseline (pre-test), post-intervention, and follow-up. Data were analyzed using SPSS software, and statistical significance was set at P<0.05. Results: Prior to the intervention, the mean self-injurious behavior scores in the intervention and control groups were 16.03±3.85 and 16.11±3.61, respectively, with no significant difference between the groups. Following the intervention, the mean scores decreased to 7.50±2.35 in the intervention group, whereas the control group showed little change (16.57±4.10), indicating a significant effect of the resilience training program. This difference remained evident at the follow-up assessment, with mean scores of 7.19±1.83 and 11.11±2.81 in the intervention and control groups, respectively. Furthermore, the physical pain score significantly decreased across the three assessment phases in the intervention group (P=0.012), while no significant change was observed in the control group. However, no statistically significant within-group or between-group differences were found regarding the variable “intention to stop self-harm” at any stage of the study. Conclusion: Resilience training was effective in reducing self-injurious behaviors and associated physical pain among soldiers. These findings suggest that resilience-based interventions may serve as an effective strategy for improving mental health and preventing self-harm in military and clinical settings.
